Job Summary:
The
Physical Therapist will be
responsible for providing Physical
Therapy services on a physician’s
referral to patients in the home
setting. He/She will evaluate function,
and render service to those adult and
geriatric patients with Physical
impairments.Job
Description:
The
Physical Therapist will perform
all or some of the following Job
description:

The
Physical Therapist will assess the
patient for functional limitations
-
Determine
a rehabilitation plan for patients which
includes long and short term goals
-
Suggest
equipment and assistive devices to help
patient attain maximum independence
-
The
Physical Therapist will provide concise
and pertinent information regarding
patient to members of the health team
including M.D.
Direct
patient care including evaluations,
treatments and recording for patients
requiring physical therapy
He/She
will assist in planning and
implementation of new clinical programs
The
Physical Therapist will select
evaluation and treatment methods
appropriate to the diagnosis, age group
and patients physical and emotional
status and formulate appropriate
treatment plans
He/She
will demonstrate essential skill
competencies in the use of treatment
modalities, equipment and protocols
Perform
re-evaluations according to department
guidelines, adjusting the treatment plan
accordingly
The
Physical Therapist will adhere to
department policy regarding timeframe,
form and content of written evaluations,
progress notes and discharge summaries
He/She
will complete all administrative and
other responsibilities in support of the
operating unit.
Minimum Requirements / Knowledge /
Skill For Physical Therapist Job

The
Physical Therapist should have 1-2
years of experience
-
Minimum of 2 years supervised
experience
-
He/She should possess customer
service skills
-
The
Physical Therapist should have oral
and written communication skills
-
He/She should have the ability to
provide own transportation for home
visits
-
The
Physical Therapist should be
able to communicate effectively with
patients, family, medical staff,
care team members, students,
supervisors and outside contacts
-
He/She should possess applicable
assessment and treatment knowledge
of preferred practice patterns
related to various patient
populations
-
The
Physical Therapist should understand
state, federal, and professional
practice laws, regulations, and
licensing requirements
-
He/She should possess basic
proficiency with a computer keyboard
-
Must
be able to demonstrate patient
centered/patient valued behaviors.
Must be able to demonstrate
knowledge of physical therapy
procedures and techniques.
Minimum Education Requirements /
Certification / Licensure For Physical
Therapist Job
-
B.S.
degree from a program in Physical
Therapy approved by the Council of
Medical Education of the AMA in
collaboration with the American
Physical Therapy Association or its
equivalents
-
Master’s degree preferred
-
Licensure: Current and valid
Physical Therapy License and
Registration to practice in the Sate
-
Current and valid State driver’s
license, auto liability insurance.
