Job Summary:
The
Occupational Therapist (OT) under the
direction of the Occupational Therapy
Manager will perform diagnostic
assessments, plan and implement specific
treatment programs and establish
appropriate discharge plans for
individual resident/patient
participation in selected tasks to
restore, reinforce and enhance
performance. Job

The
Occupational Therapist will perform
all or some of the following Job
description:

The
Occupational Therapist will
facilitate the learning of those
skills and functions essential for
adaptation and productivity
-
Assess assigned residents/patients
by evaluating data from a variety of
sources and incorporating normative
findings for older persons
-
Utilize appropriate tests and
measures
-
The
Occupational Therapist will identify
therapy diagnosis and prognosis
-
Establish realistic and measurable
short and long term goals
considering level of care and
discharge destination
-
He/She will establish realistic
individualized treatment programs
for residents/patients with a
variety of diagnoses and in multiple
practice settings
-
The
Occupational Therapist will ensure
that treatment plan is comprehensive
and is geared toward improving
functional outcomes
Ensure that treatment plan is
appropriate to meet the needs and
abilities of older persons
He/She will ensure
post-hospitalization/treatment needs
of the resident/patient are
identified and arranged prior to
discharge
Establish and regularly upgrade home
programs
The
Occupational Therapist will
prepare legible, concise and
organized written documentation
(assessments and reassessments,
treatment plans, progress notes,
plan of care) that meet external
regulatory requirements and
departmental guidelines
The
Occupational Therapist will
perform other duties as deemed
appropriate by the OT Manager.
Minimum Requirements / Knowledge /
Skill For Occupational Therapist Job

The
Occupational Therapist should be
able to assess residents/patients
with a variety of diagnoses and in
multiple practice settings
-
Adapt handling skills to meet the
needs and abilities of older persons
-
He/She should be proficient in use
of computer as it related to
documentation , billing, e-mail and
medical record review and any other
areas related to patient care or
regulatory requirements
-
The
Occupational Therapist should
demonstrate adequate skill in
discharge planning including
timeliness of discharge of services
or progression to the next level in
the continuum of care
-
Understand the mission of the
organization and the department
-
The
Occupational Therapist should be
able to adapt handling skills to
meet the needs and abilities of
older persons.
Minimum Education Requirements /
Certification / Licensure For
Occupational Therapist Job

Certified
