Discharge Planner Job Description

 

 

 

Job Summary:

The Discharge Planner, working with the treatment team, will help to coordinate the patient's transition to life after the hospital. 

Job Description:

The Discharge Planner will perform all or some of the following Job description:

  • The Discharge Planner will assess, plan, implement and evaluate the needs of patients for discharge planning activities

  • He/She will facilitate patients' move to the next level of care.          

Minimum Requirements / Knowledge / Skill For Discharge Planner Job

  • The Discharge Planner should have knowledge of discharge planning and community resources
  • Two years of related experience in acute care or other environments
  • Two to five years' experience as a discharge planner or case manager
  • The Discharge Planner should be highly organized, empathetic, capable of multitasking in a high-stress environment
  • He/She should be knowledgeable in how to handle difficult situations in a way that promotes the best outcomes for the patient
  • The Discharge Planner should have experience using Microsoft Office Products including Word and Outlook.  

Minimum Education Requirements / Certification / Licensure For Discharge Planner Job

  • The Discharge Planner must have a Bachelors Degree in Social Work or Nursing
  • Licensure in the State of practice or eligibility for licensure.
