Discharge Planner should have knowledge
of discharge planning and community
resources

Two years
of related experience in acute care or
other environments

five years' experience as a discharge
planner or
case manager

Discharge Planner should be highly
organized, empathetic, capable of
multitasking in a high-stress
environment

He/She
should be knowledgeable in how to handle
difficult situations in a way that
promotes the best outcomes for the
patient

The
Discharge Planner should have
experience using Microsoft Office
Products including Word and Outlook.
Minimum Education Requirements /
Certification / Licensure For
Discharge Planner Job

Discharge Planner must have a Bachelors
Degree in Social Work or Nursing

Licensure
in the State of practice or eligibility
for licensure.
