The Clinical Case Manager - Pool in conjunction with the Clinical Social Worker will assess and insure that all inpatients meet acute care criteria and to insure that each patient moves along the care continuum.

Job Description:

The Clinical Case Manager - Pool will perform all or some of the following Job description:

• Monitor patient length of stay against established benchmarks

• Maintain open & effective communication with the multidisciplinary team

• Ensure continuation of care in the most appropriate clinical setting.  

• Excellent verbal & written skills
• Excellent assessment skills and knowledge of disease processes within a varied patient population
• Experience as a case manager highly desired.

• BSN preferred
• Licensed in the State as a Registered Nurse.
