Management of a targeted patient
population.

Case Manager - RN will perform
all or some of the following Job
description:

Hold
joint accountability with social worker
for facilitating psychosocial and
continuing care -
Work with
the physicians and multidisciplinary
care team to facilitate efficient,
quality care and achievement of desired
treatment outcomes -
Communicate clinical information to
payers to ensure reimbursement -
Collaborate with Performance Improvement
to collect and analyze patient care and
fiscal data related to target case
types.

clinical experience

Case Management experience

Critical
thinking, advanced communication,
negotiation and collaboration skills

Organization/time management and

Knowledge
of healthcare financial and payer
requirements, as well as regulatory
agencies (CMS, Medicaid, JCAHO)
guidelines.
