Job Summary:
The
Case Manager I - Inpatient will
collaborate with colleagues in Care
Management department, service line, and
interdisciplinary care team to insure
quality driven, best practice across the
continuum of hospitalization by
utilizing the eight essential activities
of case management in direct
patient/client contact.Job
Description:
The
Case Manager I - Inpatient will perform
all or some of the following Job
description:

Perform
assessment, planning, implementation,
coordination, monitoring, evaluation,
outcomes and cost containment -
Maximize
positive financial outcomes for
designated patient populations by
intervening at key points in the disease
process.
Minimum Requirements / Knowledge / Skill
For Case Manager I - Inpatient Job
-
Five
years recent clinical nursing experience
in relevant practice area
-
Eight
years recent clinical nursing experience
in relevant practice area preferred
-
Five
years managerial or Case Management
experience preferred.
Minimum Education Requirements /
Certification / Licensure For
Case Manager I - Inpatient Job
-
Bachelor
of Science in Nursing or bachelor’s
degree in a related field
-
BSN
preferred
-
Current
licensure as Registered Nurse with the
State where job is available
-
Current
BCLS
-
Nationally recognized certification in
Case Management.
