The
Care Manager - Outpatient will perform
all or some of the following Job
description:

Work
under the supervision of a Manager and
Director, Care Management -
Serve in
an expanded nursing role to operate
within applicable patients primary care
team/office and collaborate with
hospital care managers, skilled nursing
facilities to provide a model of care
that ensures the delivery of quality,
efficient, and cost-effective healthcare
services -
Effectively communicate with internal
and external customers and care
providers in a positive and professional
fashion in order to promote
patient/provider satisfaction, quality
of care delivered and an increased
awareness of appropriate utilization.
