Job Summary:
The
Care Manager will function as a member
of the interdisciplinary healthcare team
in the provision of patient care with
the underlying objectives of enhancing
the quality of clinical and financial
outcomes and patient satisfaction while
managing the plan of care. He/She will
proactively communicate with members of
the healthcare team to ensure
appropriate medical necessity for
patients in beds; communicate with
payors and patient/family/healthcare
team regarding payor information.

Care Manager will perform

description:

Complete
a comprehensive problem identification
analysis to recognize opportunities for
intervention appropriate and realistic
for the patient/family's psycho-social,
cultural, spiritual and physical plan of
care -
Coordinate, facilitate and monitor the
plan of care for patients throughout the
episode and continuum of care to achieve
optimal clinical and financial outcomes -
Oversee
discharge planning and facilitate safe
transitions to community settings -
Serve as
liaison between members of the
interdisciplinary care team, community
providers, payor sources and
patient/family to ensure safe and
effective plans and smooth transitions
between internal and external levels of
care -
Effectively and efficiently oversee the
plan of care to optimally assure
clinical and financial outcomes.
Minimum Requirements / Knowledge / Skill
For Care Manager Job

Knowledge
of specific nursing processes, anatomy
and physiology

Excellent
verbal and written communication skills

Analytical decision making and judgment
skills

Demonstrated ability to function as a
clinical care team leader

Data
entry and word processing skills needed
-
Knowledgeable of all clinical resources
available to patients (both inpatient
and outpatient)
-
3 years
of nursing experience as a Registered
Nurse in an acute care setting
-
Pediatric
and case/care management experience.
