Job Summary:
The
Director Case Management is responsible
for overall policy &
operational responsibility for
utilization, case management and
discharge planning functions to meet
hospital strategic goals.

The
Director Case Management will perform
all or some of the following Job
description:

Oversee discharge planning, care
coordination, and utilization for
all inpatient encounters
-
Active participation in process
improvement, JCAHO preparedness,
insurer collaboration & other
hospital committees
-
Work closely with physician
leadership.
Minimum Requirements / Knowledge /
Skill For Director Case Management Job
-
Demonstrated track record of
professional leadership
-
Five
years acute care clinical nursing
experience. Additional experience in
post acute rehab, SNF, or home
health care is preferred
-
Five
years of experience in case
management and extensive knowledge
of payer mechanisms and clinical
utilization management
-
Ability to work collaboratively with
other disciplines
-
Highly effective verbal and written
skills.
Minimum Education Requirements /
Certification / Licensure For Director
Case Management

BSN
-
Health related Master's degree, and
eligibility and willingness to
attain Case Management certification
-
Licensed Registered Nurse (RN).
